ALL memberships must be approved by the Dean.

Determine if INSTITUTIONAL or INDIVIDUAL. If Individual, we need a memo from the requestor to the Dean asking for an exception. The memo must be signed by the department head before it goes to the Dean. After the Dean approves, a copy must accompany the SRFC and a second copy of the memo and SRFC must be sent to the FO.

X-coding:
If Individual membership, must be x-coded If there is any lobbying, must be x-coded
