Departmental Forms

• LAN Access and Service Request Form: This form was required by the 2003 Security Audit. All users of our networks must sign the form and the form must be on file for any follow up audits (which have occurred).

• Support and Maintenance of University-Owned Equipment in Private Homes: must be signed in conjunction with the University Equipment Sign Out form by individuals taking University-owned equipment to their homes or other non-university locations.
• A&A Group Account Form: Must be signed by all holders of a group account
• Local Workstation Administrative Access and Responsibilities: must be signed by individuals wishing to have administrative access to their computers.
• Software Sign Out: Given to people wishing to use software with work at home rights on their home computers.
